AUDITS AND INSPECTION

The safety officer will regularly conduct HSE Audits and inspection. These audits shall determine the level of compliance with HSE goals and Procedures for the project. Corrective actions for all non-conformances shall be outlined in the audit report. Each audit shall be recorded in a written report. Serious accidents or incidents will trigger a non-scheduled investigation. It is the goal of the project to close out and /or implement all negative audit findings within one week, where applicable.

Routine inspections shall be carried out in the course of the project. The inspections shall be there forms e.g pre job inspection and post job inspection. The inspection shall cover, but not be limited to the following areas; personal behavior, hazardous area, power tools hand tools, electrical equipment, hoses and connections, safety sling, housekeeping, PPE, warning lights.
Review of audit findings shall be carried out during HS&E meetings. This will be a forum where learning points are communicated to project personnel and used to improve the HS&E management of the contract.

HSE COMMUNCATIONS
The means of HSE communication shall be as in the HSE policy statements.
In the event of an emergency, the lines of communication as shown in the emergency response flowchart.

Safety meetings will normally be held regularly based on the project duration and will be chaired by the Senior Management representatives on site. The nominated safety officer is responsible for ensuring that minutes are taken of all safety meetings.

To maintain reasonable continuity of the personnel at the meetings and active participation of personal in the safety meetings a route system will operate with respect to safety representatives.

The following safety meetings will be held on site:

TOOL BOX SAFETY MEETINGS (DAILY)
The site safety Officer shall conduct toolbox safety meeting providing the workers with up to date safety information.

Site project Engineer/Foreman Attendance shall be required and records of those meeting shall be kept on file.

Toolbox safety meetings must conform to the following guidelines.

GENERAL SAFETY MEETINGS:

The company’s safety education program is composed of the following stages

NEW HIRE INDUCTION PROGRAM
On each project, sufficient time must be allocated for the safety orientation of all new hires.
Foreman participation in this orientation program shall be encouraged.
Particularly the following items shall be included in the new hired safety orientation.

MEDICAL AND FIRST AID:
The project first aid requirement will be reviewed:

SAFE WORK PRACTICES
The Site safety Officer shall ensure each foreman, when making work assignments, informs the crew or group involved of the safety practices, work methods, and personal protective equipment required.

Each foreman shall be responsible for determining that each worker has the proper protective equipment and suitable tools for the work assignment.
The site safety Officer shall familiarize the foreman with the emergency procedures developed for the project so that they may provide the leadership required to cope with serious injuries, fires, evacuations, and similar situations.

DOCUMENTATION SYSTEM

EMERGENCY/MEDEVAC PROCEDURE AND ACCIDENT REPORTING
In case of emergency, the staff of DANELEC LIMITED shall be trained to respond immediately to emergencies.

REPORTING UNSAFE ACTS OR CONDITIONS, NEARMISS
Employees will be informed that the company expects them to report all observed unsafe acts or conditions to their foreman and safety officer, who will either, resolved the problem or refer it to the site Engineer. Situations which involved imminent danger must be reported first to the site Engineer.

ACCIDENT REPORTING AND INVESTIGATIONS

Accidents cause suffering, pain and death to victims and their families, waste of time, money, materials and damage of plants and machines/equipment. It is our desire to prevent them .This Company therefore places great emphasis on the prevention of accidents. No efforts should be overlooked to achieve this golden objective. So far man has not been able to completely eliminate accidents. Hence whenever an accident occurs, its cause(s) must be determined in order to prevent a recurrence.
Every employee of the company is therefore, expected to report immediately to his foreman every accident, which occurs either to him, his fellow employee(s), or any machinery/equipment in his place of work.
Foremen are required to participate actively in the investigation of any Incidents.
Foreman accident reports are required for the following:

Near misses that had a potential for serious injury or death.
